Generally, I think it's within 2 floors of non-revealed space, however, with a large, square cavern (veterans of high x-y kingdom centers will know what I mean, if not ignore this and keep reading) I found that even 5 floors down is a valid spawn point for an enemy if there is enough darkness and the cavern *IS* revealed.
